Types of Group Health Insurance:

Health Maintenance Organization (HMO)
Medical plan in which insured’s agree to use a specific network of providers. An HMO does not have out of network benefits except for an emergency or urgent care. Some HMO’s still require referrals.

Preferred Provider Organizations (PPO)
A group of providers that are contracted at a higher rate than a provider in the HMO network. PPO rates are usually higher than HMO rates but the provider network is usually larger. With a PPO you have in network benefits along with out of network benefits. If you use a physician outside the PPO plan, your out of pocket expenses are increased. Referrals are not required.

Point-of-Service (POS)
The POS product came into being to work along side an HMO. Employees have the option to choose their physicians if they were not in the HMO network. The POS gives the employee the option to stay in network (HMO) or go out of network and pay higher copays and deductibles POS. Generally speaking rates for a POS are higher then an HMO but not as high as a PPO.  Referrals are usually not required with a POS.
